Permission, Release, and Indemnity

I hereby give my permission for the above named player to participate in the Hawaii FC Soccer Club. The player and parent/guardian(s) represent that the player is physically and mentally capable of fully participating in this program and know of no reason why participation presents a special risk of harm to the player due to the player’s particular mental and/or physical condition. In consideration of the player’s participation in the soccer programs and activities of the Hawaii FC Soccer Club, I, for myself and the player and our respective heirs, administrators and successors, intending to be legally bound, hereby release and indemnify Hawaii FC Soccer Club, its board members, trustees, officers, employees or agents from and against all claims, liabilities, damages or causes of action arising out of or in connection with the player’s participation in the Hawaii FC Soccer Club, including, without limitation, player’s transportation to/from any Program, which transportation is hereby authorized. Player and Parent Promise

As a PLAYER for the HAWAII FC, I promise to: 1. Arrive on time for all practices and games with my proper equipment (soccer cleats, shin guards, soccer socks, athletic shorts/game uniform, soccer ball, and water bottle). 2. Do my best and participate with a positive attitude. 3. Display good sportsmanship to my teammates, my opponents, my coaches, and the referees. 4. Take ownership of the Parker Ranch Arena soccer field. I will not intentionally damage the turf and I will keep the field litter-free. As a PARENT/GUARDIAN for the HAWAII FC, I promise to: 1. Notify the coach directly via text or phone if my player is unable to attend a practice. Players are expected to attend all practices. 2. Give the coach advanced notice of any planned absences from games. Unplanned absences can lead to a team forfeit and forfeits cost the club money. 3. Support the club by volunteering as a team parent, to paint the soccer field, or to supervise game day parking. I will sign up for these duties when requested at the start of the season. 4. Attend mandatory team meetings called by the club or the team coach. 5. Respect the decisions made by coaches and referees. I will speak with my coach directly if I have any concerns about my player’s participation with the club. 6. Protect the condition of the Parker Ranch Arena grounds and the safety of players by parking in designated areas only. Parent Code of Conduct

Mahalo for being a part of the Hawaii FC. Below is our Parent Code of Conduct, which we expect you to abide by, as it will be enforced by our coaching staff and club officials. Please make sure to read through and understand that this is a condition for participation in Hawaii FC. 1. As a parent, I agree to support the Hawaii FC in requiring the players to abide by the Hawaii FC Player Code of Conduct. 2. I agree to present myself as a positive role model while representing Hawaii FC at any soccer game, tournament or training session. 3. I agree to only praise players at any team game, tournament or training session. I will respect the fact that this game is for the players, not the parents, and will only provide positive communication from the sideline. 4. I will refrain from any form of negative communication or confrontation with the opposing team’s parents, players and HYSA officials. 5. I will refrain from coaching my own or any other child during the course of a game. 6. I will refrain from communicating with the referees before, during and after any game. As a parent, I will not question calls of the referee under any circumstance. 7. I will discuss any constructive concerns I have about players or the program in the following order: Coach first, Director of Coaching second and finally with the club’s Grievance Committee. 8. I understand that the coaches, managers and directors are dedicated to providing a quality soccer experience through training and team events. As a parent, I am committed to make every effort to get my child to all trainings and team events. 9. Line of communication with the coaching staff: As a parent, I will always respect the coaching staff during all games and trainings. I will never question, discuss or confront the coach during or right after any game. I understand that a ’24 hour’ cooling off period is in effect and that I will only approach the coach staff after 24 hours. I further understand that my child will first approach the coach to discuss any on-field issues. Should this not resolve the issue, I will set up a parent/player/coach meeting. I agree that if I fail to abide by any of the rules listed above that I may be subject to, but not limited by the following disciplinary action: 1. Verbal or written warning 2.
